Finally, I would like to extend my sincerest grati- tude to John Carroll, MD who until just few days ago was my Co-Editor-in-Chief. John had many personal issues in the last year and upon his request and reluc- tantly I accepted that he steps down from the journal as a Co-Editor in Chief. John contributed so much to the planning phase of the journal and I’m personally grateful that he was involved.
